Appointment requests submitted through this Site are not confirmed until acknowledged by our office. We ask that you provide at least 24 hours' notice for cancellations or rescheduling. Repeated missed appointments or late cancellations may be subject to our office's cancellation policy, which will be communicated to you directly.
